Providing short term practical & emotional support for Cambridgeshire residents over the age of 18, during a period of illness or difficulty at home.
Promote independence at home, prevent unnecessary admission to hospital, expedite hospital discharge by providing one off tasks and/or practical support over a period of 6-8 weeks. This can include but is not limited to:
- Light shopping, collecting prescriptions, light housework;
- Providing support and encouragement to help client regain confidence after hospital stay;
- Ensuring that the client is warm and comfortable;
- Advising the Coordinator if other agencies i.e. GPs are required;
- Sharing information with clients to help them find enjoyable activities and useful services;
- Feeding back to your coordinator details of any information given and passing on more complex enquiries.
